How I Treat How I treat Langerhans cell histiocytosis

چکیده
“Langerhans cell histiocytosis” (LCH) describes a spectrum of clinical presentations ranging from a single bone lesion or trivial skin rash to an explosive disseminated disease. Regardless of clinical severity, LCH lesions share the common histology of CD1a/CD207 dendritic cells with characteristic morphology among an inflammatory infiltrate. Despite historical uncertainty defining LCH as inflammatory vs neoplastic and incomplete understanding of mechanisms of pathogenesis, clinical outcomeshave improvedmarkedly over the past decades through cooperative randomized clinical trials based on empiric therapeutic strategies. Significant advances include recognition of highand low-risk clinical groups defined by hematopoietic and/or hepatic involvement, and of the importance of optimal intensity and of duration of chemotherapy. Nevertheless, mortality of high-risk patients, disease recurrence, lack of robustly tested salvage strategies, and significant disease morbidity of bothhighand low-risk patients remain challenges. Recent discovery of recurrent somatic mutations in mitogenactivated protein kinase pathway genes at critical stages of myeloid hematopoietic differentiation in LCH patients supports redefinition of the disease as amyeloproliferative disorder and provides opportunities to develop novel approaches to diagnosis and therapy. INTRODUCTION Langerhans cell histiocytosis (LCH) is a rare atypical cellular disorder characterized by clonal proliferation of Langerhans cells leading to myriad clinical presentations and variable outcomes. It usually occurs in children and young adults. It can be present with local and systemic manifestation involving skin, bone, mucosal tissues and internal organs. AIMS AND OBJECTIVES The ...

Langerhans cell histiocytosis includes a broad spectrum of clinical diseases resulted from proliferation and tissue infiltration of differentiated langerhans cells. This term includes Letterer-siwe disease, Hand-Schuller-Christian disease, eosinophilic granuloma and congenital reticulohistiocytosis that are more common in children than adults. This entity has recently been classified into restr...

Indeterminate cell histiocytosis is a very rare proliferative disorderof histiocytes which displays both langerhans cell histiocytosisand non langerhans cell histiocytosis immunophenotypic features. The majority of the patients develop multiple lesions which areclinically indistinguishable from generalized eruptive histiocytosis. We report a 14-year-old girl with multiple papules on the face,scal...
متن کاملAdult Langerhans cell histiocytosis with pulmonary and colorectoanal involvement: a case report
BACKGROUND Langerhans cell histiocytosis is a rare systemic disease characterized by the abnormal overproduction of histiocytes that tend to infiltrate single or multiple organ systems leading to significant tissue damage. It mainly affects - by order of decreasing frequency - the bone, the skin, the lymph nodes, the liver, and lungs. Gastrointestinal tract involvement is extremely rare in adul...
